I recently read alarming developments regarding an article by the National Eating Disorder Association and their imperative to remove an AI chatbot due to its dissemination of harmful advice (source). It made me think about a concerning incident where a medical chatbot, utilizing OpenAI's GPT-3, reportedly instructed a fake patient to harm themselves (source).
These instances, even if considered epiphenomena, raise critical questions about the viability of utilizing AI-enhanced chatbots in medical settings. When integrating generative AI with chatbots for healthcare purposes, several precautions need to be taken.
Accuracy and Reliability
To preserve safety but also accuracy and reliability, care should be taken to ensure the safe delivery of information. AI models responsible for generating responses must be trained on verified data using methods involving human validation, thus avoiding the dissemination of incorrect or potentially dangerous information. Regular updates and improvements to AI models should reflect the latest medical knowledge and adhere to legal guidelines.
To protect patients’ personal health information, robust security measures must be implemented, and compliance with data privacy regulations such as HIPAA (Health Insurance Portability and Accountability Act) or GDPR (General Data Protection Regulation).
Ethical considerations concerning the use of AI in healthcare should also be integrated, including the avoidance of bias, obtaining informed user consent, and transparently acknowledging AI's involvement in conversations. The AI Act can serve as a valuable framework for guidance.
Human Monitoring
Human monitoring and intervention play a crucial role in ensuring the responsible use of AI. Human experts should be involved in critical stages, including training, updating, monitoring dialogues, and intervening when necessary. One significant risk in using AI-based chatbots is anthropomorphism, where patients erroneously attribute human-like qualities and emotions to the chatbot. This can lead to unrealistic expectations, misunderstandings, and potentially harmful situations due to the lack of real human understanding and empathy. To mitigate this risk, transparent communication is crucial. Patients should be made aware that the chatbot is an AI-based program, not a human, setting realistic expectations regarding its capabilities.

Similarly, why not produce chatbot prompts and responses in such a way as to reinforce its AI nature? For example, by using language and responses that correspond precisely to the clichés referring to C3PO-style AI systems, avoiding overly emotional or human formulations.
Constant monitoring of user interactions with chatbots while maintaining confidentiality is critical. Gathering feedback and making improvements based on user experiences can increase transparency and reduce anthropomorphic tendencies.
